Health Forms

 You must schedule an appointment with the registrar to apply for a marriage license.  The bride and groom, along with a witness (over the age of 18) should all be present with photo ID and proof of address.  Please  DO NOT sign the marriage application until requested to by the registrar.  If you have any questions regarding marriage licenses, please call Cynthia Petersen at 201-391-5732, extension 222.
